New Patient Paperwork
Save time on your first visit by downloading and filling out our patient forms at your own convenience. Please bring your completed forms with you to your first appointment.
If you are unable to print forms in advance, please arrive 10–15 minutes prior to your scheduled appointment time to complete them in our office.
Your Initial Consultation (45–60 Minutes)
We deeply respect your time and make every effort to run on schedule. Subsequent treatment visits are efficient and streamlined, typically lasting between 10–20 minutes, allowing you to get back to your day quickly.
- A thorough review of your health history
- A comprehensive physical examination
- Reviewing or ordering diagnostic studies (such as X-rays or lab tests, if required)
- A professional diagnosis and a recommended, personalized treatment plan
Note: If appropriate for your condition, your first treatment session will be included in this initial visit.
